Topic: Korean Air cargo flight 8509

Summary: On December 22, 1999, a Korean Air Cargo Boeing 747 crashed in the United Kingdom shortly after takeoff from London Stansted Airport, killing all four crew members. The incident intensified scrutiny on Korean Air, already facing issues from previous accidents. British investigators identified a chain of failures, including a faulty instrument and a misguided repair attempt, leading to the crash. The crew’s passive response and communication breakdown contributed to the tragedy. Beyond being a cargo plane crash, the incident highlighted Korean Air’s struggle with safety issues.

Rationale: I chose to focus on this accident because a crucial instrument called the altitude indicator, or ADI, wasn’t working properly, and this played a significant role in the catastrophic outcome. It really emphasizes how vital it is for us pilots to have a good handle on our instruments, especially when flying in low-light conditions where we can’t rely on visual cues for the horizon. Essentially, understanding and trusting our instruments becomes paramount for safe navigation, and incidents like these highlight the importance of that awareness.

  • Teacher notes regarding my selected topic: Andres, a very good event to review. Yes, it’s not enough for a pilot to just reference instruments, a good pilot should understand what they represent and be able to cross-check in flight to ensure they are reading correctly. This is a large part of instrument training today. Psychiatric notes are a way to reflect on your practicum experiences and connect them to the didactic learning you gain from your NRNP courses. Focused SOAP notes, such as the ones required in this practicum course, are often used in clinical settings to document patient care.

For this Assignment, you will document information about a patient that you examined during the last three weeks, using the Focused SOAP Note Template provided. You will then use this note to develop and record a case presentation for this patient.

Record yourself presenting the complex case for your clinical patient.

Do not sit and read your written evaluation! The video portion of the assignment is a simulation to demonstrate your ability to succinctly and effectively present a complex case to a colleague for a case consultation. The written portion of this assignment is a simulation for you to demonstrate to the faculty your ability to document the complex case as you would in an electronic medical record. The written portion of the assignment will be used as a guide for faculty to review your video to determine if you are omitting pertinent information or including non-essential information during your case staffing consultation video.

In your presentation:

  • Dress professionally and present yourself in a professional manner.
  • Display your photo ID at the start of the video when you introduce yourself.
  • Ensure that you do not include any information that violates the principles of HIPAA (i.e., don’t use the patient’s name or any other identifying information).
  • Present the full complex case study. Include chief complaint; history of present illness; any pertinent past psychiatric, substance use, medical, social, family history; most recent mental status exam; current psychiatric diagnosis including differentials that were ruled out; and plan for treatment and management.
  • Report normal diagnostic results as the name of the test and “normal” (rather than specific value). Abnormal results should be reported as a specific value.
  • Be succinct in your presentation, and do not exceed 8 minutes. Specifically address the following for the patient, using your SOAP note as a guide:
    • Subjective: What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis? What is the duration and severity of their symptoms? How are their symptoms impacting their functioning in life?
    • Objective: What observations did you make during the psychiatric assessment?
    • Assessment: Discuss their mental status examination results. What were your differential diagnoses? Provide a minimum of three possible diagnoses and why you chose them. List them from highest priority to lowest priority. What was your primary diagnosis and why? Describe how your primary diagnosis aligns with DSM-5 diagnostic criteria and supported by the patient’s symptoms.
    • Plan: In your video, describe your treatment plan using clinical practice guidelines supported by evidence-based practice. Include a discussion on your chosen FDA-approved psychopharmacologic agents and include alternative treatments available and supported by valid research. All treatment choices must have a discussion of your rationale for the choice supported by valid research. What were your follow-up plan and parameters? What referrals would you make or recommend as a result of this treatment session?
    • In your written plan include all the above as well as include one social determinant of health according to the HealthyPeople 2030 (you will need to research) as applied to this case in the realm of psychiatry and mental health. As a future advanced provider, what are one health promotion activity and one patient education consideration for this patient for improving health disparities and inequities in the realm of psychiatry and mental health? In this assignment, you will devise a translational research study proposal using the Week 7 Translational Research Design Template [DOCX]. Your research proposal should include the following components:

Literature review: Summarize the existing literature related to your research topic including the both the EAB and applied literature citing specific gaps and how the specified theory can inform interventions that fill them.

Summarize the existing literature related to your research topic. Provide an overview of relevant theories, concepts, and empirical findings.

Step 1: Select a Special Occasion Topic 

Your speech should be for a special occasion. You may use a special  occasion from the list above, or choose one of your own. Consider the  following brainstorming techniques as you generate your speech topic: 

  • Word Association

Step 2: Consider Purpose, Thesis, and Audience 

Identify your purpose, thesis, and audience. Imagine your audience  and how you would want to convey your message to this audience. 

2a. Purpose

After you determine your topic, you should refine it into the general  purpose and specific purpose by asking yourself what you hope to  accomplish with your speech and what you hope your audience will take  away with them.  

As you plan your speech and consider the purpose, ask yourself the following questions: 

  • What do you want your audience to know? 
  • What do you want your audience to think? 
  • What do you want your audience to feel? 
  • What do you want your audience to do? 

2b. Thesis

Your thesis statement should clearly articulate the purpose and main  points of your speech. Defining a thesis is essentially constructing the  structural outline of your speech. When you have defined a thesis, you  have essentially articulated to yourself what your speech will say, what  position you will take up, and what the speech’s purpose is. 

As you plan your speech and consider the thesis, ask yourself the following questions: 

  • What is the most important message of your speech? 
  • Why should/will your audience care about your message? 

2c. Audience

When planning your special occasion speech, it is important to know  about the audience and to adapt the message to the audience. You want to  prepare an audience-centered speech, or a speech focusing on the  audience. 

The audience is the target of your speech, and effectively  communicating your theme to them is the goal of good public speaking. As  such, you should always keep the audience in mind both before and  during your time on stage. 

As you plan your speech and consider the audience, ask yourself the following questions: 

  • What is the correct context for the speech?  Where and when will it be given? 
  • Who is the audience and what are their characteristics?  
  • Why has the audience come to hear the speech? 
  • What are the interests of the audience? 
  • What does the audience know about your topic? 
  • What is the audience’s point of view on the topic? 
  • How will the audience likely react to your speech? 

Step 3: Create Notes 

Create notes or bullet points that you can refer to while presenting  your speech. You should not read your speech word-for-word from your  notes.  

Step 4: Use Clear Language 

Ensure that the language you use is consistently clear and  appropriate to the audience, which helps the audience connect with you  and your topic. Explain any technical jargon you use where necessary.  Cognitive psychologists and linguists suggest that word choice has  significant framing effects on the perceptions, memories, and attitudes  of speakers and listeners.

Overview of the Model

          Leininger’s theory of culture care diversity and universality, also the culture care theory (CCT) was developed in the last half of the twentieth century with a focus on culture and care. It has become widely recognized in the nursing domain due to its proven potential to influence the delivery of culturally competent care. The discovery of the theory stemmed from the idea that caring is a universal phenomenon, although it tends to be expressed differently across different cultures (McFarland & Wehbe-Alamah, 2019). Leininger depicted the theory as the heart and soul of nursing and as an essential component for the development of novel transcultural nursing knowledge and practices. CCT was specifically designed to guide nurse researchers in discovering new meanings, patterns, expressions, and practices related to culture, which directly, or indirectly influence the health and well-being of individuals, families, and communities.

          CCT has been successfully used to guide the provision of culturally congruent care that positively impacts the health and well-being of all. Its application forms the basis for the establishment of a transcultural nursing knowledge framework that will guide the provision of best care practices for future generations. The garnered knowledge can be used by multidisciplinary healthcare providers to transform nursing and healthcare. To date, CCT has successfully guided healthcare practitioners in explicating care meanings such that cultural care values and beliefs can be leveraged to provide a reliable and accurate basis for making culture-specific care decisions and actions (Abualhaija, 2019). It is evident from the theory that practitioners cannot separate an individual’s worldviews, cultural beliefs and practices, and social structure factors from illness, wellness, or health. This is because all the factors are closely interrelated.

Utility of Transcultural Nursing Concepts

Clinical Practice 

          CCT provides a valuable framework that when leveraged in clinical practice, results in notable process and outcome improvements. Practitioners can use the model to guide cultural assessment, with the objective of understanding a patient’s cultural backgrounds, beliefs, and values, and developing culturally competent care plans that align with an individual’s cultural context. It also guides practitioners in providing culturally-based tailored patient education to enhance understanding of the selected interventions.

Education 

          The model has been largely used to guide transculturally focused courses. Recent developments include increasing incorporation of the CCT concepts into the nursing curriculum for undergraduates and graduate nursing curricula and programs. This follows the recognition that CCT’s core constructs, such as culturally congruent care and the view of health from the perspective of patients could significantly enhance nursing competence among learners as the basis for providing culturally responsive care (McFarland & Wehbe-Alamah, 2019). An example includes designing a nursing curriculum that integrates Leininger’s sunrise model in teaching transcultural nursing.

Administration 

          The usefulness of CCT in fostering effective administration and leadership cannot be overstated. This is because such cases involve understanding and working with people from different cultures. Practices should not only be congruent with the changing cultural values but also with the societal and worldwide trends. Practical ways through which the model can be applied in this context include guiding the implementation of initiatives that promote diversity and devising training programs that enhance cultural competence. The model could also provide a basis for the review and modification of organizational policies that align with cultural diversity.

Research

          There is an increased recognition of the importance of undertaking research that is based on a nursing-specific conceptual model, as is the case with the CCT. According to Abualhaija (2019), the model has been used to design research studies and data collection methods that incorporate transcultural concepts. A typical example encompasses designing a research study that explores the impact of culturally tailored interventions on the health outcomes of a particular ethnic group.

  • The Department of Defense (DoD), notably the US Northern Command (NORTHCOM), plays a crucial role in homeland security by providing military support for domestic defense operations. NORTHCOM is responsible for coordinating military efforts to protect the United States and its territories against threats and providing support during emergencies, including natural disasters and terrorist attacks (DoD, n.d.). It collaborates closely with federal, state, and local agencies to ensure a unified response to homeland security challenges.

The National Guard also plays a significant role in homeland security. While under the control of state governors during peacetime, the National Guard can be federalized by the President of the United States to support military operations or respond to domestic emergencies. The National Guard frequently provides assistance during natural disasters, civil disturbances, and other homeland security missions, serving as a vital link between federal military forces and local communities (National Guard Bureau, n.d.).

The Posse Comitatus Act, enacted in 1878, restricts the use of federal military personnel in civilian law enforcement activities within the United States. The act prohibits the military from engaging in law enforcement activities unless expressly authorized by Congress or the Constitution. While the Posse Comitatus Act does not apply to the National Guard when it operates under state control, it does constrain the use of federal military assets, including active-duty troops, for domestic law enforcement purposes (Department of Defense, 2006). The Posse Comitatus Act impacts the use of homeland defense assets in supporting homeland security activities by establishing a clear separation between military and law enforcement functions. This separation safeguards civil liberties and prevents the militarization of domestic law enforcement. However, it also presents challenges in coordinating military support for homeland security operations, requiring careful coordination and adherence to legal requirements to ensure effective collaboration between military and civilian agencies.

Response #2 (Dylan): In the context of homeland security, the DOD plays a critical role in national security through defense support of civil authorities. When resources needed are greater than resources on hand, civil authorities may elect to call on the DOD for assistance on several domestic issues such as law enforcement support, border security, and response to natural disasters.

           NORTHCOM is one of the unified combatant commands of the United States military under the DOD. It is responsible for defending the United States within its homeland, including coordinating military assistance to civil authorities in response to natural or man-made disasters and threats. NORTHCOM collaborates closely with federal, state, and local agencies. An interesting operation that falls within NORTHCOM’s responsibility is Operation NOBLE EAGLE. NOBLE EAGLE’s primary focus is on U.S. and Canadian aerospace control. This Operation, prior to 9/11, focused more on looking outwards for threats to the United States’ Airspace. Since 9/11, the operation began looking more inward to threats that could originate within the United States. Multiple missions have been executed under Operation NOBLE EAGLE. From intercepting foreign aircraft entering American air defense identification zones (ADIZ) to escorting aircraft through temporary flight restrictions (TFR), American airspace has been made safer by NORTHCOM’s efforts since 9/11.

            The National Guard plays a crucial role in homeland security by providing a flexible and responsive force that can rapidly deploy to assist in a wide range of domestic missions, from disaster relief to counterterrorism efforts. One noteworthy aspect of the National Guard is that they may be called upon by their respective state governors to respond to civil disturbances. Under a disaster declaration in Texas, National Guardsmen were activated by Governor Abbott to assist in border security activities under Operation Lone Star. The National Guard was also called upon for support in Washington D.C. during the events that occurred on January 6th, 2021. The National Guard’s activities in homeland security and defense operations are extensive.

             The primary purpose of the Posse Comitatus Act is to limit the federal government’s power to use the military for domestic law enforcement, thereby preserving civilian control over law enforcement and preventing the military from interfering in domestic affairs. While support to civilian authorities is acceptable by means of logistical assistance, training, intelligence sharing, and providing equipment, things such as arrests, searches, and seizures are off the table for military personnel under the Posse Comitatus Act. However, The President of the United States can invoke the Insurrection Act, which allows the use of federal military forces to suppress domestic insurrections, rebellions, or other violent disturbances.

Business is in constant change with many forces that influence these changes such as global, local, legislative, cultural, etc. This week we consider how these forces impact the organization along with what managers can do when confronted with these forces.

  • Class, Surprisingly, I found several similarities when comapring the business aspects of both ExxonMobil and the Baltimore MEPS. *Please note, for the Baltimore MEPS comparison I used Army recruitment statistics as a whole as they directly impact the Baltimore MEPS.*
    1. Global Economic Trends: Externally, the most impactful global impacts for both included inflation, resource availablity and unemployment rates. In both, the economic trends have forced each company to adapt and become innovative in their future engagements. The shift in trends forces Exxon Mobil to meet societys evolving need/demand for energy and the reduction of greenhouse gas. The Army is forced to examine its failed recritment initiatives and brainstorm how to incintivize new recruits.
    2. Regulatory Changes: Changes in regulations or policies stem from political,societal, and internal perspectives that impact each business organization structure. For Exxon Mobil, global demand for the reduction of gas emmission and promotion to avoid economic hardships and market disruption have prompted the company to incentivize stakeholders. Investments in lower-emmission projects and enhance funding for technological development and implementation. For the military, policy changes directly effect recrutiment tactics and expectations. The Army has implemented reforms on specific jobs, enacted a panel that performs data analytics based on feedback from current recruiters and new recruites.
    3. Demographic Shifts: Both express the important of diversity, equity and inclusion. Each have initiatives that promote the support and celebration of the different demographics that make up the organization.
    4. Geographical Shifts: Exxon Mobil operates on six continents and continues to expand yearly. The Army is also a global organization but its primary recruitment stems from the United States. There is a vast discrepancy when analyzing the recruitment statisitics overseas.
    5. Social Perspective: Both organizations are impacted by societal perspective and reputations. Exxon Mobil’s initiative for sustainability and a “cleaner future” is seen as retroactive and not proactive. This ideal stems from Exxon Mobil’s previous stance downplaying the effects on climate change. The Army is being both proactive and retroactive. Its proactivity is in place trying to ensure that retention standards are upheld to protect the nation but its efforts can be seen as futile and retroactive as many feel there should have been programs or initiatives in place to maintain and grow the retention rates. Now, the Army is finding ways to intice new recuites by offering more enlistment bonuses, location preferences and educational benefits.
    6. Technological Developments: Both companies are centered around the innovation and collaboration provided by technological developments. The technological developments enable both to have a competitive advantage based on recruitments, progression and sustainability.

    Overall, Exxon Mobil would be the company that has a better chance of surviving based on its prioritization of competitive advantage, understanding of supply and demand and value-accretive investments. Of course, the Army is not going anywhere but to fix its recruitment issue the Army must adapt to the demographic of today. The Army should revamp its commercials to be more realistic and showcase what it has to offer that would appeal to a technological centered demographic. Having commercials that incorporate “normal jobs” would help intice potential recruits and aid in the realism that is the Army as a whole. The case study, which is an in-depth report and analysis of events, activities, or processes involving one or more individuals or a situation, is an important tool for training as it allows us to analyze real-life professional concerns without doing harm to the people who service our services.

Working psychologists continue to consult with colleagues on case studies to improve their professional competencies. While actual case studies are based on factual reports, simulated (or fictional) case studies, or vignettes, are often used to illustrate situations that could occur in practice and evaluate potential courses of action. A PowerPoint presentation can be an effective means of sharing case study details and lessons learned in a professional context.
