Creating Logic Model on Chicago’s 20 Plan of Homelessness’

Create a logic model based on Chicago’s Plan 2.0 for homelessness.

Requirements: 

1. Logic Model needs to be based on Chicago’s Plan 2.0 for homelessness.- that reading is linked here –> https://www.chicago.gov/content/dam/city/depts/fss…

2. Logic Model must look similar to the example below- and must include some color

3. The Logic Model must have 6 columns  “Inputs, Outputs, Activities, Outcomes/Impact- short term, Outcomes/Impact- middle-term, Outcomes-long-term.” To the left of the Logic Model should have the “Problem/Situation” listed as well as the “priorities.” 

4. Logic Model should be included as an editable image.
