He states he hates visiting doctors because they always try to find something wrong with him. His wife made him this appointment, but he thinks his problem is just because he is getting older. He denies any pain on urination or any blood in the urine. He also denies any difficult voiding. No foul odor is in the urine or discharge from the penis. He has tried to reduce his liquid consumption, but it has not made much of a difference. He denies any pelvic pain or rectal pain. He has not had any labs done in the past 5 years. RB is a 27-year-old female who presents to the primary care clinic for evaluation of a rash on her right arm. Possible causes range from insect bites to smallpox, but most likely the problem will lie somewhere in the middle of that continuum. The differential diagnoses tick away in our heads as we begin our evaluation of RB. The rash is vesiculopapular, moderately erythematous and localized to the right forearm. There are excoriated areas indicating scratching due to itching. She reports that the rash started this weekend. The rash itches day and night. No other family members have a rash. QUESTION

The first step of the evidence-based practice process is to evaluate a nursing practice environment to identify a nursing problem in the clinical area. When a nursing problem is discovered, the nurse researcher develops a clinical guiding question to address that nursing practice problem.

For this discussion question, you will create a clinical guiding question known as a PICOT question. The PICOT question must be relevant to a nursing practice problem (coordination of health care, assessment, education, patient support, trauma prevention, recovery, health screenings, etc.).
