  • In this case, the patient reports the absence of a menstrual cycle for the past 6months and her pregnancy test result is negative. The FNP ordered lab work to assess for a physiological cause for the amenorrhea. It is the patient’s desire to avoid hormone therapy based her beliefs. The FNP should attempt to contact the patient to discuss the lab work specifically as it relates to arriving at a diagnosis for the amenorrhea. The patient’s confusion regarding therapy even before the labs are drawn may have resulted from a communication failure between the FNP and the patient. Therapies should be discussed after the results are obtained in a shared decision-making forum. Shared decision making is based on collaboration between the provider and the patient and involves the patient’s preferences, values, and beliefs (Hoffmann et al., 2022). 

To help the patient, the FNP can contact her to discuss the case, address her concerns,  and educate her regarding the many possible causes of secondary amenorrhea. Some of the causes to discuss include psychosocial stressors, diet, strenuous exercise, weight loss, endocrine gland disorders, primary ovarian insufficiency, hypothalamic or pituitary disorders, and sequela of chronic disease (Klein et al., 2019). It is important for the FNP to listen to the patient’s concerns and to provide treatment options that align with the patient’s beliefs, preferences, and values. If the patient is amenable with having lab work, the FNP can facilitate the start of a shared decision treatment plan. The FNP should educate the patient regarding the labs and their significance. The following labs are considered to determine causation of the amenorrhea FSH, LH, thyroid panel, HCG, and prolactin (Klein et al., 2019). It is equally important to obtain a thorough personal and family health history, psychosocial history, and sexual/reproductive history to determine a possible cause of hypothalamic amenorrhea. 

This FNP will need to cultivate a trusting relationship with the patient to formulate a treatment plan the patient finds comfortable. Follow up is important to ensure the patient can access appropriate support mechanisms for her concerns regarding possible therapies.

Efficiency assesses the program’s cost-effectiveness, including reducing unnecessary utilization of resources and promoting value-based care. Social Determinants Impacting Pain Management

Social determinants of health (SDOH) play a significant role in pain management and the effectiveness of pain management programs. It takes a social determinants approach in promoting research and action on economic and commercial determinants as structural drivers of health inequities (WHO, n.d.). These determinants include socioeconomic status, education, housing, and access to healthcare. For example, individuals with lower socioeconomic status are more likely to experience chronic pain due to occupational hazards and limited access to healthcare resources. Additionally, Structural discrimination — macro-level conditions such as residential segregation — limits the conditions and well-being of less privileged groups (including the disabled, poor, women, LGBTQIA+, the elderly, and racial and ethnic minorities), which keeps these groups from reaching their full health potential (NCBI, 2022).

Furthermore, SDOH can impact the effectiveness of pain management programs. Patients with limited access to healthcare may struggle to adhere to treatment plans or attend follow-up appointments, ultimately affecting their pain management outcomes. Individuals with lower education levels may need help understanding and implementing pain management strategies, leading to better outcomes. Additionally, cultural beliefs about pain and its treatment may impact an individual’s willingness to participate in specific interventions or adhere to treatment recommendations.

THE IMPACT OF SUNK AND OPPORTUNITY COSTS

Two types of costs that can easily be overlooked when making decisions are sunk costs and opportunity costs. A sunk cost is a cost that was incurred in the past. It cannot be recovered or changed regardless of any management decisions. An opportunity cost is the cost associated with not choosing one alternative over another, and it can be measured in monetary and nonmonetary means. For example, imagine that an organization hosted a 3-day retreat to promote team building, but it was unappreciated by the employees and caused more harm than good. Once the retreat was paid for and attended, it was a sunk cost, since the managers can never return to the past and change the fact the retreat happened. The opportunity costs in this situation could be that employees could have spent those 3 days more productively by completing a project, the funds for the retreat could have been utilized elsewhere (e.g., bonuses), or a more effective retreat or team-building exercise could have been booked instead. In this Discussion, you will consider an example of a sunk or opportunity cost and its impact on an organization.
