Instructions:

Directions: Go to the NTSB website and find one accident for each of the following categories (a total of 5 separate accidents): 

1- Fatigue 

2- Thunderstorms 

3- Icing Conditions 

4- Runway issues 

case study 

5- Poor Crew Resource Management (CRM)—issues such as teamwork, communication, coordination, lack of resources, etc. 

**For each accident, address the following TWO areas: 

1- Using Microsoft Word, provide a summary of the accident. Do not copy/paste the NTSB report. Just write 1-2 paragraphs on the details of the accident. APA Paper style

2- Discuss how you feel this could have been prevented.  

3- Include the NTSB number, date of accident, aircraft type, etc.
